What does "1 Per User 1st Year Sale!" mean?

The "1 Per User 1st Year Sale!" allows you to purchase one domain at an additional discount, limited to one per customer. All subsequent registrations of a .net or .com would be at the standard registration rate. It's essentially a new customer discount.

Contrast this with "1st Year Sale!" which is not limited to one per customer, you can buy as many of those as you like at the discounted rate.
